Treatment Strategies for Management of MCA-M1 Spontaneous Dissecting Aneurysms

چکیده
The majority of MCA aneurysms are saccular lesions that arise at the MCA bifurcation region [1-5]. Aneurysms involving the M1 segment are less common, accounting for approximately12% of MCA aneurysms, and are generally small saccular lesions at the origin of the anterior temporal artery or a lenticulostriatal perforator [6,7]. MCA-M1 fusiform lesions vary significantly from their saccular counterparts [1,8-10].

Microsurgical and angiographic anatomy of middle cerebral artery aneurysms: prevalence and significance of early branch aneurysms.
OBJECTIVE To determine the prevalence of early branch aneurysms, characterize these lesions angiographically and anatomically, and determine their clinical significance. METHODS The authors conducted a retrospective review of 125 consecutive patients with a diagnosis of middle cerebral artery (MCA) aneurysm. Eighty-four patients harboring 100 MCA aneurysms were studied; 41 patients were exclu...
متن کاملA Modified Mini-Pterional (Subfrontal-Suprapterional) Approach to MCA Bifurcation Aneurysms with Minimal Dissection of the Temporal Muscle.
There are a number of different surgical approaches in middle cerebral artery (MCA) aneurysm surgery. Evolution from the classical pterional approach towards smaller modified approaches took place over the years. In the present report, we describe a new modified approach in the treatment of MCA aneurysms, which is almost exclusively subfrontal. A modified approach was used on three patients wit...
متن کاملEndovascular Treatment in Ruptured Middle Cerebral Artery Dissection Preservation of Arterial Continuity
Rupture of spontaneous dissecting aneurysms of the middle cerebral artery (MCA) is rare and its etiology remains obscure, although the risk of rebleeding is greater than with saccular aneurysms. Most reports concerning the treatment of a ruptured dissecting aneurysm of the anterior circulation involve surgical trapping or wrapping. Here, we report on a case of an MCA dissecting rupture treated ...
متن کاملSpontaneous Dissecting Aneurysm of the Intracranial Vertebral Artery: Management Strategies
PURPOSE Pathogenesis and treatment of spontaneous dissecting aneurysm of the intracranial vertebral artery (VA) remain controversial. This study was designed to provide management strategies and to improve management outcome in patients with these aneurysms. MATERIALS AND METHODS Among a total of 1,990 patients treated for intracranial aneurysms from February 1992 to June 2005, 28 patients (1...
متن کاملEndovascular management of giant middle cerebral artery aneurysms.
BACKGROUND This article reported the experience of endovascular treatment in giant middle cerebral artery (MCA) aneurysms with parent artery occlusion or stent-assisted coiling. MATERIAL AND METHODS Eleven consecutive patients with giant MCA aneurysms were included. The aneurysms predominantly involved the M1 segment in two cases, bifurcation in four cases, and M2 in five cases. Four M2 fusif...
